ToolCallHeartbeatEvent class final

Liveness heartbeat for a long-outstanding tool call (gh-1054).

Emitted every StuckToolConfig.heartbeatInterval once the call has run past StuckToolConfig.heartbeatStart — cheap, append-only sideband records that let external watchers (and the owner) distinguish alive-busy from dead. Never provider traffic.
